USB Flashdisk Cannot Be Formatted

I want to ask. I have a nexus 8 GB flash disk. When plugged in, there is a dialog box that says that "disk is not formatted". After I try to format, appear again if the "Windows Unable to complete the format". How do I fix it?  Thank you.
 

Answer:
Yudhy, there are several reasons why the USB is broken. Messages are displayed together, such as the disk is not formatted, windows Unable to complete the format, etc..
USB corrupted because Windows can not read the FAT (File Allocation Table) partition table USB. There are several solutions to address the following issues, please try one by one:  




1. Format USB using Linux.
If Windows can not format the USB, then the solution is to format the USB on another OS, in this case Linux. If only for formatting USB, Yudhy can use Linux Live CD like Ubuntu.  


2. USB format via the Command Prompt
For some cases I encountered, which damaged the USB format can pass the command prompt, do:

    
* Start> Run> type "CMD"
    
* On-screen command prompt type "format / xh:" (* h here is the USB drive, adjust to a USB drive)
function of the format / x is the "Forces the volume to Dismount first if Necessary. All opened handles to the volume would no longer be valid. "  


3. USB format via disk management
From both of the above, this way is arguably powerful enough to handle a rather severe damage to the USB (* but still the problem of software / FAT). I get these tips after tweaking the new Windows Vista which I installed. What about in XP? Of course there is. If Windows 7 I have not seen (* if other readers have already, will you please share here ya).
The following steps fix damaged usb disk management through:

    
* Right click on My Computer> Manage.
    
* Select the disk management> right click on the USB damaged
    
* Then select the format.
